We work hard to ensure all your health details and lab work are up to date, and to keep this up we kindly ask that the following be provided when submitting lab work! |
Submit your follow-up labs to Virta through our Contact Form ➡️ Submit a Request.
Required criteria for follow-up labs
- First and last name
- Date of Birth
- Date of collection
- Lab order results
Important: Lab Results May Be REJECTED If:

They are not clear/readable
- If your labs cannot be read or if they are missing any of the above required criteria, they unfortunately cannot be uploaded with your records!

Please do not send links or URLs to your patient portals as we cannot log in.
Preferred formats of follow-up labs are .PDF, .JPEG, and .PNG, but images can be accepted as well.
Note: Virta accepts follow-up labs from outside Primary Care Providers or other specialist providers, as well as our Virta ordered follow-up labs.
Automated Lab pulls
If you are enrolled in the HIE with your PCP or Endocrinologist, then our system will systematically check for updated lab work ever 24 hours. However, if you have confirmed you are enrolled in HIE with that provider and your Care Team is not seeing those results, please Submit a request to us and we can work to manually pull those labs
